Explain a problem: a ranked list of likely causes with evidence and runnable fixes, from the documents and the diagnostics bus.

Params

Param	Required	Description
`session`	yes	Session handle from `session.begin`.
`question`		What to explain: an entity and symptom (`name:Crate not visible`), a code (`ARA1302`), an asset (`@meshes/crate.glb`) or a topic (`did reload fall back to L3`, `is the frame slow`).
`doc`		Document for entity selectors (default: the one holding the entity, else the main scene).
`list`		List the explainers and example questions instead of answering.
`full`		Return the whole report as JSON (causes with evidence, checks, related). Default: the text plus the runnable fixes, which is what agents need and costs about half the tokens.
