#Common causes

• The transaction is not a tracked agent transaction, or its matching entities have no eligible review state left.
• The requested entity does not match a reviewable change.
• review.accept was called from a non-human session.

#Fixes

• Use ara review or review.list to select a currently reviewable transaction or entity.

• Have a human review and accept with their own session; agents can use review.revert or undo their changes instead of accepting them.
