Live session not found

A session-required operation received no live session handle. The handle may be absent, unknown or ended; the requested operation is rejected.

#Common causes

• The caller omitted session or reused a handle after session.end.
• A handle belongs to another project or is no longer present in this host's session state.

#Fixes

• Call session.begin with your client name, or ara_session with action: "begin", and pass its returned session on subsequent calls.

• For a per-connection stdio MCP client, let the adapter reuse its current session or begin a new one after ending it.
