Command threw an exception

A C# command threw, or a caller invoked a command already disabled by an earlier fault. The host restores journaled state and discards buffered operations for the failed call. The command remains disabled until the next successful reload.

#Common causes

• An unhandled exception in Execute or generated command invocation.
• Retrying a command before reloading after its first fault.

#Fixes

• Fix the original exception and reload the module before retrying the command.

• Use ctx.Reject for expected invalid gameplay requests instead of throwing.
